Care Innovations is a Houston-based home care and aging life care management agency, locally owned and operated since 2001. The agency provides personal assistance services for older adults — including help with bathing, dressing, mobility, meal preparation, light housekeeping, medication oversight, transportation, and companionship — alongside care management services led by NACCM-Certified Care Managers who can build a personalized care plan, coordinate providers, manage relocations, and serve as a family liaison through complex situations.
Services are available primarily in Harris County, with a focus on central and west Houston, and extend into Fort Bend County. The agency is licensed by the Texas Health and Human Services Commission as a Personal Assistance Services Agency. Engagement begins with a free consultation, followed by an in-home assessment and a care plan tailored to the client's routine, preferences, and goals. Fees are billed hourly, per session, or as a flat fee depending on the engagement.
Kris Diaz, together with business partner Cindy Smith, built Care Innovations to be small on purpose. After running a branch office for a national home care and staffing company together starting in 1988, they saw what happens when growth outpaces personal connection. Families become numbers, caregivers become interchangeable.
After watching Kris’ grandfather live independently until ninety-nine, and from a growing sense that home was where most families wanted their parents to stay, Kris and Cindy stepped away to start their own agency with a deliberate plan: stay small enough to know every client, every family member, and every caregiver by name.

That intentional scale shows up in how they work. Both owners are Certified Care Managers who stay directly involved in assessments, care planning, and family communication. When a client's needs shift, sometimes quickly, the team can respond without layers of approvals or delays. Before any first solo shift, new caregivers go through an unbilled 4-5 hour training shift at the client's home, working alongside a supervisor or existing caregiver, meeting the family, and understanding the small details that matter, like how someone takes their coffee. The details matter to Care Innovations, and nothing is left to chance.
