Care Innovations is a Houston-based home care and aging life care management agency, locally owned and operated since 2001. The agency provides personal assistance services for older adults — including help with bathing, dressing, mobility, meal preparation, light housekeeping, medication oversight, transportation, and companionship — alongside care management services led by NACCM-Certified Care Managers who can build a personalized care plan, coordinate providers, manage relocations, and serve as a family liaison through complex situations.
Services are available primarily in Harris County, with a focus on central and west Houston, and extend into Fort Bend County. The agency is licensed by the Texas Health and Human Services Commission as a Personal Assistance Services Agency. Engagement begins with a free consultation, followed by an in-home assessment and a care plan tailored to the client's routine, preferences, and goals. Fees are billed hourly, per session, or as a flat fee depending on the engagement.
Kris Diaz knew at twelve that she wanted to work with older adults, and straight out of college, went to work at a retirement community, eventually rising to executive director. Her pivot to home care came from watching a grandfather live independently until ninety-nine, and from a growing sense that home was where most families wanted their parents to stay.
After a stretch in Medicare home health and a stint at a national franchise that prioritized scale over relationships, Kris, together with business partner Cindy Smith, founded Care Innovations in 2001. The agency is small by design (around twenty-five to thirty active clients), so the team can know every caregiver well and stay close to every family. Before any new caregiver starts a case, they spend four to five unbilled hours at the client's home learning the routine, meeting the family, and understanding the small details that matter, like how someone takes their coffee.
Care Innovations also leans heavily on family education, walking adult children through what's happening with a parent, what to expect next, and how to be part of the care without being overwhelmed.
