Dana spent more than two years with a client who was HIV-positive and living with related dementia, estranged from his family over his diagnosis and his sexuality. When he chose to stop treatment and die on his own terms, she helped coordinate his transition to palliative care among his network of friends, and sat with him at his bedside the day before he died. Sheila spent years managing care for a 27-year-old skydiving instructor who was paralyzed after a tandem jump went wrong, coordinating his nursing and wound care while helping him through depression and the loss of his independence — including teaching him to play video games using head controls after he lost the use of his hands. Families and professional partners who've worked with them tend to point to the same things: responsiveness, follow-through, and the ability to hold together a complicated medical and family situation without losing the human side of it.
Concierge Health is a care management and consulting practice based in New York, founded ten years ago by Sheila Kolt and Dana Reisch. The team has grown to 12 clinicians, including the two founders, made up of social workers, nurses, certified dementia practitioners, and certified senior advisors. They take on cases involving complex medical needs — including traumatic brain injury, cancer, and paralysis — alongside behavioral health and intergenerational family concerns, and accept long-term care insurance. They serve the Five Boroughs of New York City, Westchester County, and Fairfield County, Connecticut.
Sheila and Dana both came to elder care through personal history, not career convenience. Sheila grew up connecting with long-distance grandparents over phone calls and international connections. She has spent her career close to older adults, drawn as much to their wisdom as to their care needs. Dana's was raised around chronic illness in her own family, and decided that she wanted a career built around helping people, then found her footing with older adults almost by accident, during a nursing home placement in graduate school.
That personal grounding shows up in how they built the practice. Rather than working from a standard checklist, they shape each care plan around what a specific family holds dear and around the particulars of the disease process involved. Much of the team, including both founders, has a background in psychotherapy and psychiatric hospital work, which carries into how they handle depression, loss of independence, and family conflict alongside the clinical side of a case.
