The name Solas comes from the Irish word for “light,” reflecting Coleen’s belief that care at the end of life should bring clarity, comfort, and steadiness during an uncertain time. Coleen strongly believes her own life experiences, both personally and professionally, have equipped her to be a source of quiet presence and calm for her clients and their family or professional caregivers. None of us avoids this phase of life for either ourselves or a loved one. Coleen's purpose with Solas is to ensure that every client facing a life-limiting illness receives individualized care aligning with their physical, emotional and spiritual wishes at the end of life.

The practice supports older adults and individuals facing serious or life-limiting illness who wish to remain at home while receiving coordinated, thoughtful care.
Led by a Certified Hospice and Palliative Care Registered Nurse and Certified Care Manager Coleen Nevin, the practice offers care planning, family guidance, and coordination with healthcare providers to support care transitions, symptom management, and decision-making. Services include comprehensive assessments, individualized care planning, family meetings, and advocacy between clients, families, and medical teams.

It did not matter, day or night, Coleen was only ever a phone call away. She worked quietly, anticipating [the client] and our every need. While death is certainly a part of life, it is a part of life that none of us are prepared to deal with; especially losing someone that should have had many more years with us. There are a lot of things to be considered as the end nears that families cannot fathom on their own. Not the least of which is knowing what to do in those most precious of last moments and accepting that the end has come. Coleen’s most gracious gift is her ability to soothe the heart-broken with the most majestic guidance. There are no words sufficient to express what Coleen did for us all.
